**Issue Resolved** Clinical Safety Issue - Vision+ eGFR Calculator

This issue has been resolved in Vision+ SIS10590.

An issue has been discovered with the use of the recently updated eGFR calculator made available in the Vision+ product from December 2019. An issue has been discovered with the use of the recently updated eGFR calculator made available in the Vision+ product from December 2019. The eGFR calculator may be used to estimate renal function in patients. The issue identified is specifically in relation to calculations performed using the Cockcroft-Gault equation and results in an incorrect creatinine clearance result being recorded for patients whose actual body weight is greater than their ideal body weight. This may affect decision-making for prescribing certain drugs.

Vision is currently investigating this further and a fix to the calculator that will fully resolve this issue will be deployed to all Vision sites that have access to Vision+ by the end of March 2020. In the meantime, Vision requests that users of the GFR calculator suspend its use with immediate effect and use an alternative version of the Cockcroft-Gault equation creatinine clearance calculator (e.g. https://www.mdcalc.com/creatinine-clearance-cockcroft-gault-equation).

Vision is working to identify patients that may be affected and will communicate directly with practices with advice on any actions required to mitigate the risk to these patients.
